My micro SD card cannot be detected. Help?!

So, I own a 32GB Kingston micro SD card and a Samsung Galaxy S3. All the media that is stored in the micro SD card does not exceed 5GB and yet the remaining space I have on the micro SD card is less than 10GB. (It was the case when I checked in on my phone and on my macbook). I though something was weird so I removed the SD card (safely) from my phone, used an SD card adapted and plugged it into my macbook and backed up all my data. Then I plugged it back into my phone and formatted the SD card. It was empty and I removed it safely once again to transfer all my previous data back onto the SD card. It was peachy when I saw that my SD card had more than 20GB of space available. This was until my phone started popping up notifications saying that the SD card was removed unexpectedly while I was using my phone which would have been impossible because the phone casing was on the whole time. Thinking it was nothing, I removed the SD card safely again and plugged it back on. I worked for a while until the notifications show up again. It kept happening until one day my phone wouldn't even detect my SD card. Neither does my macbook when I plug the SD card in again with the adapter. So I resorted to performing a factory reset on my phone. Still, nothing has changed. Can someone please explain to me and perhaps help me solve this problem? Did I do something wrong by formatting the SD card because it lost some important aspect to making it possible for it to be detected?
